    The Power of Attorney for Health Care form makes it possible for adults in Wisconsin to authorize other individuals (called health care agents) to make health care decisions on their behalf should they become incapacitated. This process ensures that an individual's wishes about their health care is known and respected. Trained ADRC staff is available on Wednesday mornings from 9 a.m. - noon to: • Assist Kenosha County residents with completing their Power of Attorney for Health Care • Support the process of Advance Care Planning

    A social group (not a support group), for people with memory concerns and their loved ones. Share stories and socialize with others who have concerns about their memory. Being active physically, socially and mentally is GOOD for the brain! This ADRC event is sponsored in partnership with the Alzheimer's Association, Brookdale Senior Living and Hospice Alliance.
