The federal government requires that, “Any facility (business, municipality, school, farm, etc.) that utilizes any of the 364 Extremely Hazardous Substances as listed by the U.S. Environmental Protection Agency (EPA) above certain quantities or any Hazardous Chemicals as defined by the EPA above a certain quantity are required to report this information.

Information needed is: Name, address, phone number, Municipality, whether they are insured, how much water/sewage they had and whether it was on the first floor or in the basement.
