IRIS in Kenosha County
Introduction
IRIS is a Medicaid long-term support Self-Directed Waiver Program sponsored by the Wisconsin Department of Health Services available in Kenosha and other counties across Wisconsin that provide Family Care. If you choose IRIS you can manage your own services within a budget determined by your needs.
What is Include, Respect, I Self-Direct (IRIS)?
IRIS allows you or your guardian to choose, control and design their own support and service plan to meet your functional, vocational, medical and social needs. With IRIS, you have control over the type of services you receive at home or in your community residence. IRIS is grounded in the core values of self-determination:
- Freedom to decide how you want to live your life
- Authority to purchase services within a specific budget
- Support to organize resources in ways that are life enhancing and meaningful to you
- Responsibility for wise use of public dollars and recognition of the contribution you make to your community
- Confirmation of the important role of self advocates
Who is Eligible for IRIS?
To be eligible you must be age 18 or older, and have a physical or developmental disability or be elderly and frail. You must meet state functional requirements and must be financially eligible for Wisconsin Medical Assistance (Medicaid.)
