Transition and the Aging and Disability Resource Center
The role of the (ADRC) is to assist you to plan for your future by providing information and assessment regarding available services.
THIS IS ACHIEVED THROUGH 4 BASIC STEPS
- Options counseling for community resources. Option counseling includes referral and information regarding:
• Vocational services
• Guardianship
• In home supports
• Recreational and leisure activities
• Continued education - Information on applying for Social Security and Medicaid.
- Assessments for functional and financial eligibility.
- Referrals for Family Care, Partnership or IRIS if eligible.
Most programs have their own eligibility requirements . To satisfy these requirements you may need the following documentation:
- Medical records
- Financial information
- M-Team results/records
- Additional legal documents
- School records*
* IMPORTANT: Schools do not keep records indefinitely. We recommend getting your records prior to or immediately following graduation.
LEGAL STATUS CHANGES
At age 18, legal responsibilities become your own unless a guardian is appointed for you. You can plan how medical or financial decisions are made if you can't make them.
The ADRC can assist with information and re ferrals for the following:
- Guardianship
- Power of Attorney for Healthcare
- Power of Attorney for Finances
- Special Needs Trust/Estate Planning
THE PROCESS
PROGRAM ELIGIBILITY & ASSESSMENT
The ADRC will assist you in obtaining services you qualify for but may not be receiving. You may request an assessment to determine functional and financial eligibility by calling the Aging and Disability Resource Center at (262) 605.6646.

WE CAN HELP
As you turn 18 you will want to know your transition options. The Kenosha County Aging and Disability Resource Center (ADRC) can help you identify your goals for the future and the support you will need to live as independently as possible.
The ADRC provides long term care counseling to adults with intellectual and physical disabilities, their families, friends, caregivers, guardians and other concerned parties.
