Community Living Program
The Community Living Program can help you avoid nursing home placement by obtaining community-based services. The program is designed for older adults who need assistance with their day-to-day activities and do not qualify for Medicaid.
Our goal is to give you tools you need to make choices. Community Living Care Coordinators can help you plan in order for you to remain in your home independently, healthy and safe.
What’s Important to YOU?:
- What things are important in your life that gets you going every day?
- What do you wish you had more time for?
- List the things that don’t happen all the time, but make you feel good about your life?
- What matters most in your life, such as family relationships, friends, your career, your pets, spirituality?
- Are there people in your life who help you every day?
- Does your current health condition cause limitations for you in your life?
Person-Centered Approach Where Individuals:
- Determine your personal wants, needs, and goals
- Make decisions about your supports, services, and personal budgeting
- Tailor your personal plan along the way to meet any changes or needs
You and the Community
Talk to a Care Coordinator about services and supports based on YOUR individual care plan and budget.
Flexibility and choice in long term care services and supports complement our primary goal for YOU to remain at home for as long as possible.
The Community Living Program has Care Coordinators who:
- Are available short-term to meet with participants and provide assistance and information as needed
- Can help arrange services and supports consistent with one’s immediate and long-term goals
- Will provide follow-up to see how things are going, whether services are working out as planned and making adjustments as needed
