How does someone get into Brookside Care Center?
Once admitted to the facility can I ever leave?
Brookside admits several residents for short-term stays. Many come for a rehabilitation/medical stay and return to their home or apartment.
What do I have to do to be admitted?
A physician must give orders to admit someone along with a History and Physical exam and any specific doctors orders such as medications, diets, etc.
How much and what types of personal furniture may we bring in?
Brookside encourages you to make your family member's room more "homelike", however it is easy for a room to become over crowded and unsafe for moving around. (Not to mention that we clean a wee bit more than you probably do at home.) See the room first, and decide what personal furniture will fit and enhance the atmosphere as simply as possible.
Does Brookside have activities in the evenings and the weekends?
Brookside Care Center proudly offers programs in both the evening and on weekends. Activities vary with time so please check the activity calendar located in each room and on the units. Everyone is encouraged to attend.
Do you plan community outings?
Brookside Care Center has its own bus that can carry up to fifteen passengers. The bus has a wheelchair lift and six spaces for wheelchair users. We average seven community outings a month that include shopping, fairs, picnics, out to eat or just going for a drive.
What if I don't like attending social activities?
There are many ways to enjoy your days at Brookside without attending social events. Some self-directed pleasures may be enjoyed are bird aviary, our eighty gallon fish tank, computers, TV rooms and our therapy dog Nova. You can enjoy our beautiful grounds with many patios and walking paths. Or enjoy the privacy for your own room reading, watching TV or visiting with friends and family. At Brookside there is always something going on.
How does someone get into Brookside Care Center?
To make admission inquires, please phone (262) 653-3800, Monday through Friday, from 8:00am until 4:30pm. The social worker will assist you in gaining the necessary information to determine if Brookside Care Center, a skilled nursing home, is the appropriate setting for your loved one. After providing detailed admission information and tour, the worker will have you complete a pre-admission form, which is the first step to await an appropriate opening in the nursing home.
