Through the support of your Rehabilitation Team, we will help you achieve your personal rehabilitation goals. You will set the pace and we will be there to support and guide you through your challenges and successes.

You’ll become acquainted with the services and support systems available to you. We want you to know that we are here to help. Your questions and comments are welcome- they will help your rehab team evaluate how well they are meeting not only your needs but those of your family. We are committed to providing the highest quality of rehab. care available anywhere regardless of the setting. With your commitment and participation, we’ll be there to support your progress.

Brookside Care Center and MJ Care, Inc. work together to provide quality type rehabilitation program for your loved one. Our expertise includes physical, occupational and speech-language therapy. Our competence is ensured through the selection of clinical professionals who actively participate in continuing education programs. Our commitment to continuing education ensures up to date knowledge of technologically advanced treatment.

What to expect of your rehab program
Shortly after your entry to rehabilitation program, you will meet the members of your Rehabilitation Team. In order to appreciate the progress you will make during the visit, it is important for team members to perform a comprehensive evaluation…a description of your “starting point”. This evaluation, with the assistance of you and your family, will help determine your special rehab goals and individual treatment plan. Along the way, you, your family and your rehab team will modify these goals based on your progress and wishes. Changes are accomplished through regular patient care conferences.

When your treatment plan is established, you will receive a daily schedule. Efforts will be made to balance your therapy sessions with rest periods and to accommodate special recreational, religious or family events. You will also be exercising during your rehab program. The most commonly asked question is, “Why must I exercises?” Exercise is proven to decrease the adverse effects of your medical condition. Exercise will increase your strength and endurance so that you can begin to regain independence and control of your life.

We welcome your family members or caregivers to become closely involved in your therapy program and goals. At your invitation, they are encouraged to visit your therapy sessions to support you, share in your progress and help prepare you for your discharge. Your pet may also visit (check with the facility administrator for permission). Special social and educational programs are also available for you and your family.

Key Players on your Rehabilitation Team
YOU! ... You will be well supported during your rehabilitation experience by a team of highly trained professionals. Of course, your attitude, commitment and active participation are essential to regaining your independence and function. You are the center of your Rehabilitation Team.

Your Family / Caregivers
Family, friends and caregivers are essential members of your support team. Their active involvement in your rehabilitation experience will facilitate discharge plans and a better appreciation of your abilities and needs.

Your Doctor
You doctor has ordered your rehab program for you. Throughout your rehabilitation program, your doctor will receive regular progress reports from your therapists and nurses. Any change in your plan of care must be approved by your doctor. Close communication between you, your doctor and other rehabilitation team members is of the highest priority!

Physical Therapist (PT)
Physical Therapists and Physical Therapist Assistants restore muscle, strength, activity endurance, joint mobility, safety, ease of transfers, and walking, as well as promote healing and relieve pain. Specialized rehab programs exist for prosthetic (artificial limb) training, wound care and fall prevention. PTs’ and PTAs’ use various treatments such as moist heat, ultrasound, electrical stimulation and whirlpool.

Occupational Therapist (OT)
Occupational Therapists and Occupational Therapist Assistants will assess and provide individual programs to help you perform activities of daily living such as bathing, eating, dressing, grooming, and homemaking. Treatments may focus on such areas as thinking and problem-solving, coordination, sense of touch and range of movement to perform functional activities.

Speech Language Pathologist (SLP)
Speech Pathologists are essential professionals who help you develop or regain communication skills as well as improve understanding and interaction skills. Specialized services are provided to improve swallowing and breathing functions related to stroke or other neurological conditions. Specialized communication systems may be used to enhance your communication ability.

Your Rehabilitation Schedule
Your day usually begins with taking care of your personal needs with the help of the nursing staff, followed by dressing and grooming with a nursing assistant or occupational therapist.

Your rehab schedule will depend on your needs, the type of therapies ordered and your comfort level during therapy sessions. You will also have the opportunity to coordinate therapy sessions around your personal interests and activities. Remember, your family is welcome to attend therapy sessions if you would like to invite them.

Care Meetings/Staffings
Your rehab team, with the exception of your physician, are present at these scheduled meetings. You and your family are encouraged to attend, because you are the focus of your rehab team. These meetings are designed to review therapy progress, develop mutual goals, and discuss discharge options.

Your Departure
From the day you are welcomed aboard, your care is constantly monitored and evaluated to provide for a smooth transition from your rehab program to your future residence. We will help you and your family coordinate your departure within a time frame that meets your unique needs. Preparations for discharge may also include a visit to your home or other setting to insure that you and your family have the skills and support needed to successfully function after you leave us. Your may receive a home program and/or specific instructions or recommendations which will assist you in the transition. However, our support for your care is only a phone call away. Don’t hesitate to call us if you have any questions that require additional information after you’ve left. We welcome your suggestions as a way of assessing and continually improving the care we provide to you and your family.
