Health Check Exams - Medical Assistance Recipients
A health history and screening providing: physical assessments, height, weight, health counseling, nutritional education, Hgb testing, immunizations, urine testing, vision and hearing screening, lead testing, and referrals to other health care providers as needed.
Well Child Exams
- Provides the same services as a Health Check exam.
- $25.00 Fee for exam.
- $9.00 Administration fee per immunization is requested.

Nurses can provide a copy of our assessment.
We cannot sign outside agency forms.
Sports physicals are not provided.
